(3)(b) The sink was not clean and sanitized that is used for preparing bottles.(g) Licensor observed a bottle heated in a microwave. (j) Bottle is not stored prepared and unserved bottles in the refrigerator (i) Licensor observed provider not throwing away contents of any formula bottle not fully consumed within one hour.
